This was adventure #7 in the SoftSide Adventure Series (formerly SoftSide Adventure of the Month Club). It was published as a bonus on SoftSide CV and DV (the cassette and disk versions) with the instructions in SoftSide Selections, issue 44, vol. 6, no. 11, September 1983, pp. 28-29 and encoded hints in SoftSide Selections, issue 45, vol. 6, no. 12, November 1983, p. 27 (Atari version).
This is called 'Madhouse' in the game itself and 'Mad House' (two words) in the instructions and SoftSide Selections. The latter were written by SoftSide staff, not the original author, so the former is considered to be the correct title.
Jim Gerrie has created a version of the game for the TRS-80 MC-10.
